How do you plan your goals

First disclosure, I am working on business goals management app, but my point is not to promote it here...

Mainly for my learning how do you plan your business goals. For example, you want to reach a certain MRR, do you break it into an entire plan or think about it from time to time? More interesting to me is why you handle your goals the way you handle them?
